Summary
Examines: a series of construction errors which causes a devastating flood that brings Chicago to a standstill; two buildings, halfway around the world from each other, that collapse from the same type of shoddy construction methods--14 years apart; a cockpit warning system that malfunctions, causing a fiery, fatal crash before takeoff; three boilers that explode on a Mississippi riverboat, killing thousands of Union soldiers; and a deadly accident that traps hundreds in a smoke-filled Alpine tunnel with inadequate ventilation.
